Ideal Candidate Criteria

Bachelor’s degree relating to behavioral/physical health and one (1) year of experience or Associate’s degree relating to behavioral/physical health and two (2) years of experience or High School Diploma/GED and four (4) years of experience in a behavioral/physical health field.  

Primary Function: 

This position is responsible for coordinating behavioral Health and physical health services of enrolled members. Care Coordinators will ensure that members receive quality integrated services to promote behavioral and physical recovery of member and increase independence in the community.

Job Duties

  • Knowledge of community resources to assist members in social determinants of health.
  • Provide care coordination services to support members which includes but is not limited to ED, hospitals, family members, probation, detention centers, PCP, specialists, natural supports, etc.…with other health care facilities to ensure positive outcomes.
  • Provide appropriate discharge planning and follow up with members within required timeframes.
  • Partners with team members to develop appropriate care plans to manage chronic illness.
  • Assist members with the process of determining eligibility.
  • Collaborates with member, Health Home, and physical health team members for service planning.
  • Manage referrals, when needed, to appropriate agencies required to assist the member in achieving goals and objectives identified in their Care Plan.
  • Demonstrates cultural awareness of members/families in assessment, service planning and service delivery.
  • Establish and maintain relationships with identified Medical and Behavioral Health service providers.
  • Assist with data collection and generation of member registry reports.  Works collaborative with identified internal staff on data collection for HEDIS and other key indicators.
  • Ensure concurrent or same day documentation in Electronic Health records.
  • Participate in Performance Improvement/Continuous Quality Improvement activities, as assigned.
  • Facilitates ART/CFT in collaboration with the multidisciplinary teams to address progress toward meeting goals and achieving outcomes, barriers, celebrations, and next steps toward recovery.
  • Completes required trainings within prescribed time frames.
  • Ensures members are engaging with their PCP to complete their care management treatment plan or preventive care services.
  • Evaluates the medical necessity and appropriateness of care; creating positive outcomes.
  • Development of service plan that is individualized and recovery based, and assessments as assigned.
  • Provides services based on member need which may include evening, weekend, and holiday coverage outside of normal business hours.
  • Provide transportation services to members based on medical necessity which may include out of town transporting.
  • Timely response to communication requests within 24 hours or as identified.
  • Support members in scheduling and/or accompanying members to appointments.

Minimum Qualifications

Bachelor’s degree relating to behavioral/physical health and one (1) year of experience or Associate’s degree relating to behavioral/physical health and two (2) years of experience or High School Diploma/GED and four (4) years of experience in a behavioral/physical health field.

Must be 21 years of age or older, possess a valid Arizona Driver's License, and be able to show proof of automobile insurance coverage. Must be eligible to obtain a Fingerprint Clearance Card upon hire.  Must become certified in First Aid, CPR and CPI (training provided to those not already certified).

Physical Requirements:

  • Use a Personal Computer or telephone up to five hours a day
  • Frequent long, irregular hours
  • Frequent and/or prolonged travel
  • Occasional reaching, bending, stooping, kneeling or crouching
  • Frequent standing or moving throughout facility
  • Light physical effort (infrequent lift/carry up to 25 pounds)
  • Ability to operate a motor vehicle
